
In a landmark collaboration aimed at revolutionizing healthcare delivery for older adults with multiple chronic conditions, Sensorum Health and Intermountain Health have announced a strategic partnership. Leveraging Sensorum Health’s cutting-edge remote monitoring technology and Intermountain Health’s expertise in clinical quality improvement, the alliance seeks to enhance health outcomes and primary care access for vulnerable populations.
The partnership comes at a critical juncture, as the prevalence of chronic conditions among older adults continues to rise, posing significant challenges to healthcare systems worldwide. Andrew Hotchkiss, CEO and Co-Founder of Sensorum Health, highlighted the pressing need for innovative solutions, stating, “Older adults with multiple chronic conditions face considerable barriers to managing their health effectively, often resulting in avoidable hospitalizations. Our goal is to empower patients with the tools they need to proactively manage their conditions and access timely care.”
Central to the partnership is Sensorum Health’s advanced remote monitoring technology, which enables real-time detection of early signs of emergent health issues. Coupled with Intermountain Health’s patient-centered approach to care delivery, the alliance aims to prevent unnecessary hospitalizations by intervening at the earliest stages of disease exacerbation. “Our solution integrates novel remote monitoring technology with specially-trained clinical staff to identify the earliest signs of emerging health issues and triage patients to their primary care doctors for treatment,” Hotchkiss explained.
Intermountain Health, renowned for its commitment to clinical excellence and patient-centered care, views the partnership as a strategic opportunity to enhance care delivery for vulnerable populations. By targeting value-based care patients enrolled in the Castell Health Salt Lake Valley “House Calls” program, the initial pilot project will focus on individuals with conditions such as congestive heart failure and chronic obstructive pulmonary disease, who are primarily homebound.
